ADVANCED TRAUMA LIFE SUPPORT

Advanced Trauma Life Support® (ATLS®) is a 2 1/2 day course sponsored by the American College of Surgeons©, (ATLS®) prepares physicians to identify and respond to life-threatening traumatic injuries. Additionally, (ATLS®) is not a stand-alone exportable course; it is conducted in conjuntion with C4.

The content and skills presented in this course are designed to assist doctors in providing emergency care for the trauma patient.  The ATLS Course provides the essential information and skills doctors may apply to identify and treat life-threatening or potentially life-threatening injuries under the extreme pressures associated with caring of patients in the fast-paced environment and anxiety of the resuscitation room.

COMBAT CASUALTY CARE COURSE (C4)

C4 is a basic 8-day tri-service continuing medical education program designed to enhance the operational medical readiness skills of physicians, nurses, physician assistants, dentists, and other specialty healthcare providers intended for initial entry medical professionals with minimal field exposure. C4 prepares junior tri-service medical officers with the knowledge critical in conducting Level I & II healthcare operations in an austere, combat environment. C4 provides training in leadership skills, field medical knowledge and the practical information needed for direct medical support of tactical units under combat conditions. The course includes 3 days of professional programs at the Defense Medical Readiness Training Institute, Fort Sam Houston and five-days of intensive field training at Camp Bullis, Texas.

Students attending C4 have the opportunity to receive certification in Advanced Trauma Life Support® (physicians and oral surgeons; dentists, physician assistants,and nurse practitioners may attend as auditors), the Trauma Nursing Core Course (nurses) and Pre-Hospital Trauma Life Support (other healthcare providers). Participants are awarded continuing education/credentials upon successful completion of these courses.

At the conclusion of the professional programs, the course moves to the field as students rotate through various training stations in an operational environment. Areas of instruction are varied and complex, ranging from bandaging and splinting to Level 1 & Level 2 care in the Tactical Simulator for Military Medicine (Tac-SiMM). Students encounter scenarios simulating combat environments where they must perform the duties of their respective career-field positions as well as the duties of first responders. They also have exposure to various real-world scenarios, as well as a myriad of medical readiness topics taught through didactic lectures.

Intended for general and orthopedic surgeons, the course was developed in response to 1991 Gulf War reports that surgeons and other healthcare providers lacked sufficient training and experience in trauma surgery. To remedy this, the Deputy Assistant Secretary of Defense for Health Affairs designated proficiency in combat surgery training as an “essential mission.” The Combat Trauma Surgical Committee (CTSC) was created in 1996 to meet this mission and to coordinate training efforts to develop and sustain DoD trauma care providers.

The EWSC is a standardized, measurable and reproducible training program that will give surgeons and other health providers exposure to critical life saving skills necessary to sustain the force, fight and win in the 21st century.

Formerly known as the Homeland Security Medical Executive Course (HLSMEC)

This program focuses on working within the National Response Framework to prepare, respond, and recover from an all-hazards incident.  Participants gain knowledge & skills using expert presentations, table-top exercises & professional interactions.  Major speakers are representatives from various military and governmental agencies including Federal Emergency Management Agency (FEMA), Federal Bureau of Investigation (FBI), U.S. Public Health Service (USPHS), Department of Homeland Security (DHS), Department of Health and Human Services (DHHS), and the U.S. Northern Command (US NORTHCOM).  State and local topics are presented by State and local emergency management and Public Health officials.  Additionally, we invite a leading industry partner to discuss corporate preparedness and integration efforts locally and at the State and national levels.

At the conclusion of this course attendees will be able to identify key components that build the foundation for an effective medical response to an all-hazards incident, and  identify the process to ensure that a comprehensive national medical response system is brought together and it coordinates all necessary response assets quickly and effectively to an all-hazards incident.

Intended audience: HLSMPC is open to DoD Active Duty/Reserve/Guard, US Public Health Service and US Coast Guard medical department officers and local, state, and federal healthcare professionals and emergency managers.  Students in previous courses included designated command surgeons and senior staff/deputies, senior staff (doctors, nurses, admin, operations, logistics, public health/preventative med), NDMS/FCC staff, Medical Emergency Operations Center (EOC) staff, Joint Regional Medical Planners, ESF-8 related personnel, and other Emergency Preparedness officials.
It is highly recommended that each participant accomplish the following Courses prior to HLSMPC attendance:  FEMA IS-100: Introduction to Incident Command System, I-100; FEMA IS-200: ICS for Single Resources and Initial Action Incidents; FEMA IS-700: National Incident Management System (NIMS), an introduction; FEMA IS-800: National Response Framework, an introduction, and Defense Support to Civil Authorities (DSCA), Phase I.  FEMA courses are available at http://training.fema.gov/IS/.  DSCA Phase I course is available at http://www.usarnorth.org.

JOMMC familiarizes and develops senior healthcare personnel to serve as Joint Task Force Surgeons’ and Staff members/.  The mission of these individuals is to:

• Provide Health Service Support (HSS) recommendations to the JTF Commander
• Coordinate HSS operations in the Joint Operations Area
• Establish and maintain operational medical facilities
• Coordinate Coalition and Civilian medical interoperability 

JOMMC is made up of just-in-time training to provide Joint HSS instruction punctuated with scenario-based tabletop exercises that reinforce course objectives. 
JOMMC is intended to provide entry level instruction on Joint HSS.  JOMMC is not intended for those students who have previously attended the JTF Surgeon Course or Joint Medical Planners Course (JMPC).
JOMMC is open to Tri-Service Active Duty, Reserve, Guard, US Public Health Service, and US Coast Guard medical department personnel, as well as foreign national medical department officers. Students will leave the course with the capability to provide doctrinally sound, operationally integrated leadership in joint health service support operations successfully meeting current demands on combat service support units.  Moreover, they will be able to successfully meet the current demands of operational and tactical commands.

PHTLS is a 2 or 2 ½-day course sponsored by the American College of Surgeons © and the National Association of Emergency Medical Technicians. PHTLS is conducted in conjunction with the Combat Casualty Care Course (C-4) and during exportable training sessions.

DMRTI supports a program that sets up a unit as a PHTLS Training Site, allowing unit personnel to sustain readiness training without dependence on DMRTI returning to the unit. The target population is tri-service medical personnel in active duty or reserve status.

The purpose of the PHTLS course is to provide the student with the underlying beliefs that are the foundation of the program - that definitive care cannot be provided for the critically injured trauma patient in the field.  The PHTLS program attempts to increase each participant's understanding and skills in pre-hospital trauma management. This increased understanding of kinematics, pathophysiology, systemic impact, and intervention techniques will result in improving the assessment and treatment of the multi-system trauma patient and offer a perspective into the patient's individual needs.  The PHTLS program enhances the philosophy that care of the multi-system trauma patient is unique and requires specific needs that may exceed traditional modalities.  Therefore, the PHTLS program stresses the need for Rapid and Accurate Assessment Identification of Shock, Hypoxemia Initiation of Intervention Techniques, and Rapid and Safe Transportation

TNCC is 2 1/2 day course sponsored by the Emergency Nurses Association. The target population is military Nurse Corps Officers.

The purpose of TNCC is to present a basic level and psychomotor skills associated with implementing the trauma nursing process. The psychomotor skill stations facilitate initial integration of psychomotor abilities in a setting that stimulates trauma patient situations. It is the intent of the TNCC to enhance the nurse's ability to assess, rapidly and accurately, the patient’s responses to the trauma event. It is anticipated that the use of the knowledge and skills learned in the TNCC will ultimately contribute to a decrease in the morbidity and mortality associated with trauma.

Emergency Medical Response to Weapons of Mass Destruction

The Emergency Medical Response to Weapons of Mass Destruction (EMRWMD) develops and sustains CBRNE competencies for DoD medical personnel whether they are assigned to an operational assignment, civilian mass casualty or humanitarian disaster response.  EMRWMD better prepares military and interagency medical personnel for augmentation and sustainment to local, state, and federal authorities in response to a domestic or overseas incident. 

Coordination of this course is through a partnership between DMRTI and the 2nd Medical Brigade.  Oversight of the EMRWMD course will by provided by DMRTI while 2nd Medical Brigade will be the executive agency.  EMRWMD is a three-day course, held at various locations with the main training installation being at Camp Parks, California.  The course is divided into didactic classroom lectures, field training exercise, and table-top exercise. Topics include chemical agents, biological agents, radiological materials, high-yield explosives, triage and field management of casualties, psychological/behavioral response, decontamination, personal protection, and consequence/risk management.

Each of these topics is re-explored in field training exercises and table top exercises where the participants utilize their knowledge through hands-on applications.
